Ben Yorgey Memorial Award 2012
Wednesday, January 4, 2012 at 9:28PM Please look over the following list. If you know of a young person with special needs that would fit the criteria, we encourage you to download the nomination form, fill it out and return it to Ben Yorgey Foundation, 5771 Buckeye Rd., Macungie, PA 18062.
Application deadline is March 2, 2012.
Criteria Necessary for Consideration of Nominee
1. Person nominated is to be someone who loves people, school, life and is an active participant in the community.
2.Registered with Lehigh County Office of Intellectual Disabilities or Carbon County Developmental Services.
3.Leaving the school system at age 21.
4.At time of nomination, not receiving any waiver.
5.Recommendation accompanying nomination form must come from an educator, parent and/or community member.
